Effective immediately, forms to be filled out can be dropped off in person on Wednesdays ONLY.

The receptionist will check to make sure all of the appropriate information is filled out and it is signed appropriately.  Forms put under the door will not be addressed.

COVID Vaccine clinic booking at HCHC

HCHC is currently administering Pfizer (12+ yrs) and Moderna (24+ yrs) vaccines in office.  Please indicate if first or second shot in reason for visit.

HCHC Introduces ONLINE BOOKING!!

By November 1, 2021 all HCHC physicians will have online booking access. Most are already active.  Appointments only appear on the portal if they are available.  

New appointments will be opened between 4:00pm and 5:00pm, the day before to allow patients to book their virtual visit.

Appointments scheduled with a physician other then your primary care physician will be denied.  Nurse Practitioners and Nurse visits, as well as Allied Health continue to be booked over the telephone.

This is a great alternative to waiting on the telephone for an appointment.


Medeo Health

HCHC is pleased to provide patients with access to Medeo.  Medeo is a patient platform that allows physicians to email patients directly regarding test results and requisitions, or things like notes.  If you are interested in joining us on Medeo, click the link:

Once you have joined, make sure that the office has your email address on file.  An email can only be used by ONE person.  That means everybody in the house, children included, must have their own email address.  Google allows for child emails managed by parents, which is a good suggestion for this service.

Medication Renewals

Effective September 1, 2018 fax requests for Dr. Allen Greenspoon, Dr. Yellin, and Dr. Zalter will no longer be considered.  You MUST book an appointment for ALL renewals.

Renewals for narcotic or controlled medication must have a booked appointment and will not be considered over the fax.  Appointments for these renewals must be booked in advance.  There is no guarantee that you will have your medication on time if you do nor pre-book these appointments.

*** Medication Alert ***

The Province has implemented a new program called Patch For Patch.  If you are currently on any form of pain patch, ie: Duragesic or Fentanyl, you must return your old patches to receive new ones.  This is effective immediately!



The emergency clinic is for medical emergencies only. We do not do prescription renewals, give test results, or forms during these times...For more information please see Policy & Procedures.

At this time, emergency clinics are being handles on a booked basis, not walk-in.  You must call the office for direction.  DO NOT appear in person.

Please do NOT use walk-in clinics or housecall doctors.

Pay Parking is in effect.

Rates:

$2.000 for each 1/2 hour or part thereof to a max rate of $6.00
