About Family Health Teams
Family Health Teams are a key piece of the provincial government's plan to build a health care system that promotes good health, reduces wait times and improves access to care. In Family Health Teams, family doctors work with other health care professionals such as nurse practitioners, nurses, mental health counselors, dietitians and pharmacists to see more patients and to keep them healthy.
 
About the Hamilton Family Health Team
The Hamilton Family Health Team (HFHT) located in downtown Hamilton, serves part of the Local Health Integration Network 4 (LHIN 4 Hamilton, Niagara, Haldimand and Brant) region and is the largest Family Health Team in Ontario.
 
Improving the Health Care System
The vision of the HFHT is simple - to build “a healthier Hamilton”. To achieve this vision the HFHT is focused on providing collaborative primary health care options that are both exceptional and comprehensive, and that will significantly improve the health and quality of life of individuals within the local community.

Services Offered as part of the Hamilton Family Health Team
As a member of the Hamilton Family Health Team, we are able to offer you access to Mental Health Counselors, Dietician Services, Pharmacist, as well as the use of our Registered Nurses and Nurse Practitioner.  There is also access to several centralized services, such as therapy groups, physiotherapy, Psychiatry, and many more offered at 123 James Street South.
